Daycare Contract



 

            

                          JODY'S DAYCARE CONTRACT

                      UPDATED ON  OCTOBER 1 - 2009  

         As a competent daycare owner working with children one through seventeen years of age.   We here at our daycare have strict rules regarding our safety equipment i ensure all smoke detectors, fire extinguishers, carbon monoxide detectors and emergency lighting are in there proper places and in good working order. We plan and execute monthly fire and tornado drills we also make sure our staff is skilled in well skilled in this getting the children to safety in a emergency situation is our top priority. We make sure all of our toys and play equipment is in safe and good working order. If something is broken ,dirty or not working it is either repaired or replaced promptly. We all practice good hand washing techniques here. When preparing meals. coming in from outdoors,after using wash rooms and diaper changes and before and after meals. We will always treat all of the children in our care with the utmost affection and respect. We will always strive to provide the children with a great learning environment . We are able to provide the children with a excellent preschool program. We also provide a computer center with quiet area where the children are able to do home work assignments etc. If this is needed. We have a large yard with big play area where they are able to work on large muscle development. We can easily adapt our daily schedule to accommodate all children with special needs with help from parents,teachers and professionals if needed. I believe all of these children as well as there parents will be pleased with our program. In conclusion; we here at Jody's daycare we will always strive to give your child the most experienced and loving care possible.                                                                                                              When arriving to daycare:

1.  Please make sure to bring a change of clothes for your little one.

2. *Please have your little ones in a car seat when you arrive. Missouri Law states children under 80 lbs and under 4FT,4 Inches need to be restrained in a car seat unless they are over 8 years old. 

3.  Please have your little ones fully dressed when you bring them to daycare.

4. Parents must sign their children in upon arrival and sign them out when you pick them up.

5. Please do not bring your child to daycare when they are ill or have a temperature.

 

Hours:

Jody's Daycare is open 24 hours a day MONDAY - SUNDAY CLOSED ALL HOLIDAYS. 

If your hours and days change, from week to week, please provide a schedule every SUNDAY  by 7pm. 

 

 There is a  enrollment fee of   $50.00 PER FAMILY.

 

There is a supply fee for every family.     The rate is  $20.00 per child AND $10.00  each additional child no more than $30.00 per family .  This will be applied to gas to field trips.  Visitors that come to work with children and supplies needed for your children at daycare. extra  that i offer  Hamster,fish,vistor's zoo and music lady. 

Prices:

No overnight care for children that don't sleep all night. I need to sleep too.

RATES FOR  CHILDREN 1- 3 year old

 

5 DAY WEEK  IS  $  90.00

4 DAY WEEK IS $76.00

3 DAY WEEK  IS  $54.00

2 DAY WEEK IS  $36.00

1 DAY WEEK IS  $ 18.00

13 YR  OLD AND  OLDER WILL GET  40 HR FOR $50.00 A WEEK  FOR BEFORE  AND AFTER SCHOOL ONLY. 

 

I will charge a hourly rate for parents under 4 hrs, this will be $2.00 a hr per child.  ANY OTHER HRS ARE CHARGED AS RATE ABOVE . Any extra hours before or after shift  are charge the same for regular parents. ( AS LONG AS I HAVE AGREED).  DROP INS DO NOT GET THIS RATE.    

 

 

Contracted hours;      Are defined as the hours of care agreed upon and stated in your financial agreement. For example: If your work hours are 7:30 AM to 4:30 PM. Your drop off/pick up times might be 7:00 am to 5:00 PM.

 

 There will be an additional fee for any hours exceeding the contracted hours.  This fee will be $5.00 every 30 minutes. You must pick your child up on time.

 

*A Missouri daycare license allows a certain number of children at all times depending on the hours and help. Any overlap with extra children caused by parents picking up their children late could result in fines or punishment to the daycare.

 

 THERE  WILL BE NO DROPPING OFF BETWEEN THE HOURS OF  1:00PM TO 230PM  unless they are quiet .  CHILDREN ARE   ASLEEP. 

 

Financials:

I report all money paid to me to the IRS as income. You will be given a receipt every time you pay. At the end of the year I will provide each parent with a signed statement showing  how much you have paid. It will include my Tax, ID number so that you may take the child care tax credit. There will no amounts given over phone. Every year you must sign a invoice for my records. Call ahead if your need it in a hurry.  I will take up to 24 hours to give this to you. If you owe me money, forget it  you will have to pay, to get a receipt.

 

 HOLIDAY ARE MY DAYS OFF WITH PAY.   CHILDREN  UNDER 3 $18.00  AND  OVER 3   $12.00   HERE  HERE A LIST OF HOLIDAY THAT I WILL BE CLOSED. ALL PARENTS  PAY HOLIDAY EVEN IF YOUR NOT SCHEDULE. 

1. CHRISTMAS EVE-CHRISTMAS DAY-DAY AFTER  1 DAY CHARGE.

2. NEW YEAR EVE AND NEW YEAR DAY CHARGE 1 DAY.

3.  MEMORIAL DAY AND WEEKEND  1 DAY CHARGE

4.  INDEPENDENCE DAY AND AFTER 1 DAY CHARGE.

5.  LABOR DAY AND THE WEEKEND  1 DAY CHARGE

6.  THANKSGIVING DAY  AND  BLACK FRIDAY  1 DAY CHARGE

7. CHRISTMAS I WILL BE CLOSED FOR AT LEASE 3 DAYS  1 DAY CHARGE. 

NO THERE WILL BE NO MORE FREE VACATION DAY OFF NO SICK DAY OFF.

All other day off will be posted. 

 

Late/early Pickups/drop-offs:

There is a late fee of $5.00 every 30 minutes past your scheduled time.

Parents are Responsible to call in if they are running late dropping children off. If you have not shown up after 3 hours, your time slot could be filled. If you don't show up and don't call.  You will  be charge your normal rate. ( no refunds). If your planning on going to work  later call me also.

*Parents will still be accountable for payment for their contracted hours, whether your child is here or not. (Parents receiving state paid daycare will have to pay out of pocket for missed days)

If your hours and days change, from week to week, please provide a schedule every sunday by 7pm.

 Payments:

All Payments (including CO-payments) are due in advance. Payment is due on the first day of the week that the children are attending. 

  If payment is not received on  the first day of child(ren) attendance, you will be charge  $25.00 on the second day + the fee of childcare.  (You will be stop at the door for payment).

I do take credit cards,cash,check's.   I do not charge extra for using the credit card machine.  Parents are only to use it when you current with payment. 

 

REFUNDS:

There will be no refunds of money paid for childcare even if your child does not attend.   

Bounced check

Will cost you a $25.00  fee. So please make sure you have money before you write the check.

If payment is not received  on friday before the week. On monday  you have a $25.00 late fee.   You will be stop at the door for payment.


 

 

 

 

  

State paid daycare:

In order for parents to take advantage of this program, you will need to provide paperwork showing you have been approved.  A phone call from your worker will be acceptable.

  

 

 

Jodys Daycare provides  Breakfast, lunch, afternoon snack and a dinner each day.

 

 

If children have a special dietary need, parents will be responsible for bringing substitute foods. (If im unable to help with the special diet.) 

 

The following is a sample days menu.

Breakfast: Oatmeal, banana and Milk

Lunch: Grilled cheese sandwich, fries, tomato soup and Milk

Snack: String cheese, crackers and water

Dinner: Baked chicken, mashed potatoes, green beans, roll and Milk

Learning activities:

I do many fun and educational activities with the children. I love to teach and I have preschool  curriculum ,computer educational games ,educational and fun field trips  in and out of the home.   Lost of good learning.

Children are sponges and learning through play and activities at a young age is extremely important for later growth and development.

I Have added a music teacher she will be comming in once a month, to our home for other teaching  opportunity, for your children. the zoo  will visit us with different animals everymonth for the afternoon children , will be  going to gymnastics studio , libuary, Bass Pro , anything else that we can find that fun and entertaining.  At Easter we will be having the Easter Bunny visit and at christmas  we will be having Santa Claus, we will be having children birthday parties with parents help.  Any other holiday celebration we will be asking for donations. right now our field trips are on hold.   

 

Newsletters:

Important information will be communicated through these handouts. Please read them carefully. The newsletters are intended to inform you about all the children under my care, other parents, community needs, and what your children learn while they are having fun.

 

Extra Clothing:

All children, no matter how old they are, need to have a FULL change of clothes; this includes underpants, socks, and season appropriate tops and bottoms. Parents are responsible for providing at least one change of clothes. Always send a coat and shoes because we do go outside often.

 

Other Supplies:

For bedding: A  blanket is all they will need. Send special bed toys if needed.  To put in there cubbies.

 

Toys brought from home:

 YES, children can bring Toys from home.

I will not replace toys broken by another child at daycare . 

Bring at you own risk. 

   

Nap time:

Children 1-5 years of age  that are required by law to have a nap time. From 1:00 to 2:30pm we have quiet time. No loud playing. the older children will watch a movie or do puzzles. Children are not made to go to sleep but they are required to stay on there cots and have quite time for a least 30 minutes. I need parents to work on this at home so their child is not waking up the other children during nap time.

 

Separation From you Child/open door policy:

I encourage parents to pop in or call often to hear your child's voice. You can come at anytime to visit your child (no inside visiting during nap time) You may arrange for the children to leave daycare for a two hour period during 1 to 3 PM.

Safety:

Safety is the main concern in my daycare. I have childproofed my home extensively.  Poisons and medications are stored in locked cabinets and are out of reach of children. I have fire extinguishers and carbon monoxide detector,  hard-wired smoke detectors and emergency lighting.  Electrical outlets are capped when not in use.

*PARENTS ARE RESPONSIBLE FOR THEIR OWN CHILDREN ONCE THEY ARRIVE. MAKE SURE CHILDREN DO NOT GO OUTSIDE without YOU.

THIS DAYCARE WILL NOT BE HELD RESPONSIBLE FOR ANY ACCIDENTS WHICH OCCUR FROM PARENTAL NEGLECT.

 

IMMUNIZATIONS:

It is the policy of Jodys Daycare that all children enrolled must be immunized. All immunizations must be kept up to date. Any child found not up to date will have 24 hours notice to receive the required immunization. The provider can refuse care to any child/children if required immunizations have not been received. I require a new  Medical Examination (Infant/Toddler/Pre-school) form sign by the doctor)   every year up to age 5 yr old . 

 

Sanitation:

I use a bleach solution to sanitize toys. Big toys are cleaned once a week; small toys, each day as needed. I do practice and preach hand-washing. Children must wash hands after each bathroom visit and before and after every meal.

Emergency Procedures:

We will practice emergency tornado drills once a month and fire drill once a week or more per shift. In case of fire we will leave the house and meet at the  side walk in front of the house. I will not have time to call parents, just 911.

 

Smoking:

Jodys Daycare is a smoke free facility. Please do not smoke inside or by the entry door when you arrive. My husband does smoke but only outside. Smokers who work here will be required to smoke outside.

 

Discrimination:

I will accept children of all sexes, nationality, origin, race, or disability. (assuming Jodys Daycare is able to accommodate the disability) There will be no practice of discrimination of others inside of my daycare by other children, or parents. I encourage parents to talk with their children about tolerance and the importance of not making fun of others.

Outside Play:

We will be outside 2 to 3 times per day, depending on weather. This means anywhere from 30 minutes up to 3 hours. (Fresh air is great for everyone)

 

Abuse and Neglect Reporting:

I am required to report all cases of suspected abuse and neglect to the proper authorities. This will include children not being properly restrained in their car seat or seat belts. I have to make a report if your child is hurt at your home as well as mine. Let me know if your child was injured at home and when it happened. I will contact parents immediately if a serious injury has occurred, all other injuries or boo-boos will be notified the next time I see you.

 

Discipline:

Only redirection and time-out in a room by themselves  with the door open is the discipline that we use here at jody daycare.  If I am unable to handle your child I will remove the child from daycare. If a child has any behavior problems at all, you will have to have a behavior plan from doctor on file. You will be on a daily trial period in this case. If your child is hurting other children he or she will have to be removed from the room. If being removed from the room doesn't work or they start hitting or yelling at me etc. You will have to come get your child. If your child has caused injury bad enough to cause a hospital or doctors office visit, your child is not welcome to come back.

 

Potty Training:

I feel potty training works best when started around 2 ½ years old. Children in the training stage MUST be able to pull their own pants up and down, and understand when they are wet. Pull-ups or plastic pants must be used. when on field trips  3 year olds need pull-up or a diaper. 

 

Vacation:  you will have to pay for your slot.   

 

Funeral leave:

 

If you experience a death in your immediate family (including your own parents/siblings), you will be given a three day grace period without a charge, if you choose to keep your children out of daycare. I will require a copy of the funeral announcement to waive your daycare fees. This is simply our way of supporting you and your family though this difficult time. 

 

*** Jody's Daycare is about family and I want to support you and your family through life's happy experiences as well as your difficult times. Your child's physical and mental health are dependent upon us working together as a team!

 

 

Medications:

Please do not leave any medicine in your child's bag, hand-deliver it to me. We don't want other children to get this medication and take it by accident I will administer medication only if I have a signed statement from the parents giving me permission. It is the parents responsibility to bring over-the-counter medications with their handwritten instructions for me. Prescription medications will also need to have their name on it and must not be expired.

 

SIGN IN SHEETS ARE LOCATED IN THE  Entry hall way.

It is the parents reasonability to sign in each and every time. Not doing so will cost you the price  of the meal I don't get reimbursed for. the meals are $2.11 a meal.  So make sure you take the time to sign in and out every day. No reminders anymore. If your a state paid parent you must sign or state doesn't pay .  Then you will owe for a day of daycare,also.

 

  

Your daycare could be lost for any of these reasons.

1. If the state says at anytime during services that you have lied to me

2. Failure to comply with the policies set forth in this booklet

3. Failure to complete required forms includes preschool doctor form.

4. Nonpayment of childcare fees  due friday before.

5. Failure to show up for 3 days in a row without communication

6. Not having your child up to date with shots (once a year I will ask for a new copy of shot records children that are not in school will need a new preschool forms signed by doctor.

7. Blatant disrespect towards provider or providers family, or other families in my daycare

8. Any child that is breaking my toys everyday, hitting and/or intimidating other children. Also if a child causes harm to put another child into the hospital.

9. Bringing a child to daycare that has head lice. Take care of this yourself. Let me know if your child has head lice so that I can check the other children I do not want an epidemic in my daycare.

10. Consistently late pick ups. (I understand about car problems, or unexpected meetings. Please call ahead. I will not tolerate late pick ups or early drop offs on a daily basis)

11. Consistently early drop off (I don't care if you need to drop other children off or want to go shopping before work.) You have to set up this prier to this time.

12. Masking a child's illness with medication.

 

Illness policy:

THESE ARE THE ILLNESSES THAT ARE NOT ALLOWED HERE AT THE DAYCARE. DO NOT TRY TO HIDE ILLNESSES. COMMUNICATE WITH ME ALL INFORMATION ABOUT YOUR CHILD'S HEALTH.

1. Severe diarrhea (not caused from food allergy and occurs more than once)

2. Vomiting (not caused by choking)

3. Headaches and/or stiff neck (not caused from sleeping wrong.)

4. Fever of over 99 degrees. (unless I have a doctors note explaining your child has high temperatures)

5. Sore throat or trouble swallowing

6. Severe cough (sounds like Croup or Whooping cough)

7. Exposed to the strep virus.

8. Pink eye (tears, redness of eyelid lining, irritation, followed by swelling or pus discharge)

9. Head lice (*I have a NO egg policy. If you child has eggs or head lice they will have to leave. I will need the empty bottle of medicine used to treat the child. I will check your child everyday for 3 weeks. If I find and egg I will call you to redo their hair. No Excuses. Head lice spreads very fast. I will give you a list of things to do to prevent this matter and other advice on how to treat the problem)

10. Chicken Pox (until sores are scabbed over)

11. Whooping cough

12. Yellowish skin or eyes

13. Urine that is tea colored or unusually dark

14. Difficult or rapid breathing

15. Infected skin sores that are crusty, yellow, dry or gummy

I will look over your child as they arrive. If I have a feeling they are sick or scratching their head, I will check. I will expect you to pick up your child within 30 minutes if I feel it necessary to send them home. Phone numbers are required to be up to date. A doctors note is required for children to be allowed back in the daycare.

 

 Everyone is on Probation:

I give all new families/children a two-week trial. I want to ensure that the family and I can work together as a team. Some parents refuse to cooperate and follow rules. Some children lack respect of others and deliberately hurt other children while parents do not care. I want cooperative, involved parents that are willing to work with me on all issues in this contract. A child with behavioral problems is on a day to day probation with a behavior plan in effect from the doctor a head of time. No exceptions. 

   

 

 

I understand everything I have read and will follow the contract, to the best of my ability.
